Cauliflower Ear in BJJ — Causes, Prevention and What to Do

The thick, swollen ear you get from grappling isn't a badge of honor; with care, you can prevent it. Why it happens, prevention with ear guards, and why you need to drain swelling quickly.

You've seen people who've done a lot of jiu-jitsu or wrestling with thick, deformed ears, right? That's cauliflower ear. Some people treat it like a "badge of honor," but if you know about it and take care, you can prevent it. Once it hardens it's hard to undo, so learn about it ahead of time.

Why does it happen?

Repeated friction and impact on the ear cause blood and lymph to pool between the cartilage and the skin. If it isn't drained, it hardens that way and becomes lumpy and deformed. It often happens when your ear gets scraped while defending guard passes, in scrambles, or when you're stuck under knee on belly.

Prevention

  • Ear guards (headgear): the surest prevention. If your ears swell easily, just wear them.
  • Reduce friction: slightly change your head angle in positions where your ear gets scraped.
  • Early response: if your ear feels hot or swells up soft after rolling, ice and compress it that same day.
If it's swollen, drain it fast

If your ear is puffed up, it's best to get it drained as soon as possible by a doctor (ENT or plastic surgeon). Some people drain it themselves with a needle, but the infection risk is high, so it's not recommended. Neglect, then hardening, then permanent deformity: that's the sequence. Timing is everything.
